Claim Management/Insurance Policies
We expect the patient to take responsibility for knowing the benefits of their insurance plan. Our office may offer some assistance, but we cannot be responsible for any information that turns out to be incorrect.
Our office will respond to insurance requests for information, but will not be obligated to take action on behalf of patients against an insurance carrier for collecting or negotiating an insurance claim.
Insurance benefits are estimates only, not a guarantee of coverage. If your insurance company does not pay, you are financially responsible for payment of all fees and charges.
We DO bill your insurance company directly for you.
To date we are contracted with all HMOs associated with Physicians' Medical Group of Santa Cruz (PMG). That includes:

- Health Net HMO
We are also contracted with:

- Coventry
- Cigna
We are waiting for final paperwork for Guardian, Interplan, and Central Coast Alliance for Healthcare, which includes all the Healthy Families programs, even if they are Blue Cross based.
