You may schedule an appointment by calling during regular office hours.

An online request form will soon be available if you would like one of our staff to contact you to schedule an appointment.

What to bring to your appointment

  • your insurance card
  • your co-payment
  • an updated list of all your prescriptions from ALL of your prescribing doctors OR the actual bottles
  • a list of any over-the-counter medications, such as aspirin, vitamins, and/or supplements.

We are now an “open access” office

That means that about half of our appointments are available as same-day appointments, allowing us to accommodate our urgent care patients.

We encourage you to mark your calendar when you need to be seen for follow-up care and call that day. Pre-scheduled appointments are still available, especially for physicals and procedures.

Cancellations and Late Arrivals

We reserve your time slot especially for you!!! Therefore we appreciate your prompt notice if you are unable to keep your appointment or if you anticipate being late.

Please give us at least 24 hours notice for cancellations if possible, so that we can offer your time slot to another patient.

If you are running late, the staff will check our schedule, and if time allows, we may be able to see you. Otherwise, you will need to reschedule.

No-Shows

Missed appointments are costly. If you do NOT notify us of your cancellation, we reserve the right to charge $25.00 for regular appointments and $50.00 for no-show physicals.

As a courtesy, we will attempt to call you the day before your appointment to remind you, but it is your responsibility to remember the appointment.

Please make sure we have your current information, so that we can reach you!

Transferring your medical records

Please check with Dr. Solomon before transferring your medical records from another office. Most offices charge for this service and it is often unnecessary to transfer your entire record. Sometimes arrangements can be made electronically.
